Eyelash Lift & Tint

By signing this agreement, I consent to having my lashes permed/lifted and tinted by Folly Beach Medi Spa. Although every precaution will be taken to ensure your safety and well-being before, during, and after your eyelash lift, please be aware of the following information. As part of the procedure, there are risks involved such as eye irritation, eye pain, eye itching, and discomfort. I agree that if I experience any of these conditions that I will contact my technician and consult a phyician at my own expense if needed. I understand that a lash lift will curl my natural lashes and that depending on length and strength of natural lashes, results may vary.

Aftercare Instructions:

I understand and agree to the aftercare instructions provided by the certified professional. I understand that if I do not follow proper aftercare that it may damage my natural eyelashes to which the lash technician is not responsible for.

No sauna/steam or swimming the first 24 hours
No mascara the first 24 hours/No waterproof mascara at all
No pulling, picking, or rubbing of the eyelashes
Use only *oil-based makeup remover to remove eye makeup
Use the aftercare oil *daily to keep lashes healthy

Cancellations:

I understand that my appointment time is reserved just for me. A late cancellation or missed visit leaves a hole in their day that could have been filled by another client. I understand that my esthetician requires 24 hours notice for any cancellations or changes to my appointment. I understand that I will be charged 50% of my scheduled service fee if I am unable to give 24 hours notice or no-show my appointment.
I understand that if I am more than 15 minutes late to my appointment that it may need to be rescheduled. I also understand that if the technician is able to fit me in that my appointment may be shortened but I will be charged the full amount.

I give permission for pictures to be taken before, during, and after my service and understand that those photos may be edited and used for marketing and training purposes.

I agree to and understand the no refund policy and agree that if I am unhappy with my service that I will contact lash technician within 48 hours so any issue can be resolved.

I understand the risks associated with this procedure and will let my technician know immediately if I experience any discomfort.

I understand that proper aftercare is my responsibility and is important for maintaining my lash lift and the integrity of my natural lashes.
